The following wards are outside of the area of benefit of the three Parochial Charities:
In the north west – Woodberry Down, Brownswood, the western half of Stamford Hill West
In the west – Clissold, the western half of Stoke Newington
In the south west – Haggerston, Hoxton West, Hoxton East and Shoreditch
For Individual Applicants
- Confirm that you live within the area of benefit of the charity to whom you are applying. Check using our Postcode Checker and Map.
- Confirm that the items of furniture or white goods you are applying for are in line with our guideline prices on the How To Apply page.
- Confirm that you have a third party professional supporting organisation in place before you complete the application form.
- If you do not have a suitable third party supporting organisation, then please contact the East End Citizens Advice Bureau on 020 8525 6350 and ask them to be your third party, or do contact us on 02033 977 805 for support and we will then link you up with a third party organisation.
- You must also have two professionals from charities or organisations as your two referees, one of whom must provide a written reference letter.
- Complete the Individual Application Form, completing all of the fields on the form, and then submit the form to us.
- You must state all of your income, rent, council tax, savings and debts, plus the income of any other household members, and also provide evidence of these to your third party organisation.
- You must send us one proof of your current home address, such as utility bill, statement, license or tenancy agreement.
For Third Party Supporting Organisations to Individual Applicants
- Confirm that the Applicant lives within the relevant Area of Benefit, using our Postcode Tracker and Map.
- Support the Applicant to complete the Individual Application Form.
- Please ensure that all of the fields are completed on the form, including: 1. Name and date of birth of the applicant’s partner (as applicable). 2. Children’s names, dates of birth, and any financial contributions they make (as applicable). 3. Weekly income and benefits, plus partner’s weekly income and benefits (as applicable). 4. Details of Housing Benefit, Council Tax Benefit and heating allowance. 5. Does the applicant have any savings or debts. 6. Information about the applicant’s current health (as applicable). 7. Whether they have applied to the Discretionary Crisis Support Fund. 8. Name of third party referring agency, contact name, address, email, and phone number. 9. Names of two separate professional referees, including their job roles, addresses, emails, and phone numbers e.g. religious leader, GP, support worker, healthcare professional, housing provider, local Councillor, charity, employer etc. The referees must not be related to the applicant.
- An applicant may complete the application form independently on their own, provided they have first obtained your consent to be their third party, and have completed all the fields on the form.
- Provide the following additional Supporting Documents: 1. At least one piece of evidence of the applicant’s current home address e.g. utility bill, statement or tenancy or license agreement. 2. A reference letter from you as the Third Party Supporting Organisation, on your headed paper, confirming the applicant’s current circumstances of hardship and your relationship to them. 3. Web links or other evidence of the cost of each individual item that the applicant is applying for e.g. links to pages in an online stores catalogue. If specialist items are applied for then a written quotation must also be provided from the company e.g. carpets, flooring, etc.
- Please confirm that you are able to accept a grant payment into your own organisation’s bank account, in the event of an Individual Grant being awarded. Grants are normally paid by BACS payment.
- If a grant has been awarded, complete and return a Monitoring Report, which we will send to you, plus receipts for the items purchased.
For Individuals Applicants seeking a grant from the Hackney District Nursing Association (HDNA)
- Confirm that you are a nurse or midwife or trainee nurse or midwife.
- Confirm that you are a Hackney resident who is living within the London Borough of Hackney.
- You can apply for a grant for your training and ongoing education.
- You can apply for a grant for medical equipment and aids.
- You can apply for a grant if you are in need of financial support.
- You must also have two professionals from organisations as your two referees, one of whom must provide a written reference letter.
- Complete the Individual Application Form, completing all of the fields on the form, and then submit the form to us.
- You must state all of your income, rent, council tax, savings and debts, plus the income of any other household members, and also provide evidence of these to your third party organisation.
- You must send us one proof of your nursing or midwife qualification, or letter from your Health governing body, confirming that you are in training.
- You must send us one proof of your current home address, such as utility bill, statement, license or tenancy agreement.
- If a grant has been awarded, complete and return a Monitoring Report, which we will send to you.
For Organisations and Charities
- All projects must either be delivered in a venue within the area of benefit of the relevant charity to whom you are applying, or you must evidence that at least some of the people who are attending your project must be living within the area of benefit of the relevant charity.
- All applications must be made to the relevant charity on the Project Application Form, and all of the fields on the form must be completed. Strategic grant applications are also made using the Project Application Form, but please state that it is a strategic application on the form.
- Please ensure that your project falls within the general project areas and aims of the relevant charity.
- Your application should be endorsed by two referees who know the work of the organisation. These referees should be totally independent of the organisation and should not be a relative or partner. Suitable referees include prominent members of our local community, other charities, local councillors etc.
- You must attach a copy your latest annual report and accounts plus a detailed project budget.
- You must attach one Reference letter form either of your referees, on their organisation’s headed paper.
- You must confirm that you have an up to date safeguarding policy in place for the relevant groups with whom you are working.
- If a grant has been awarded, complete and return a Monitoring Report, which we will send to you.
